USA Patriot Act Customer Identification Policy
In order to open a new account or add a signature/co-maker to an existing account the following information will be collected. Copies will be placed in the member’s file.
- Current Photo Identification (For an account opened for a minor child, the identification of the person opening the account will be required)
- Mailing Address
- Physical address, if different from the mailing address. (Verification of the physical address must be provided
- Birth Date
- Social Security Number
